Insurance Filing Information

Because of the complexity of some insurance plans, we will work with you to determine, as closely as possible, your actual cost. We request that you pay co-payments, deductible and any non-covered charges when you arrive for your appointment. We will file all insurance claims for you.

Uninsured/Self-Pay Patients

For patients who are not using insurance for their office visit, a $350 deposit will be due at check-in. This deposit will be applied to the actual charges at check-out. If the visit charges are over $350, the balance will be due at check-out. In the event the actual charges are less than $350, the difference will be refunded at checkout.

Forms of Payment

Co-payments and balances are due at the time of service.

Financing is available through CareCredit for hearing aids and medical procedures. Go to the CareCredit website for more information www.carecredit.com or see our office staff for details. We accept Visa, MasterCard, American Express, Cash or Personal Check.

Buckhead ENT does not accept Medicaid, i.e. CareSource (Medicaid Managed Care), WellCare (Medicaid managed Care), Peach State (Medicaid Managed Care), Amerigroup (Medicaid Managed Care) ; Pathway, Pathway Enhanced, and Pathway Guided Access HMO networks (off Exchange), Pathway X, Pathway X Enhanced, and Pathway X Guided Access HMO networks (on Exchange).

 

Possible Out of Pocket Cost

Please be aware that, in order to determine the correct diagnosis so that the most appropriate treatment can be recommended for your medical condition, your physician may need to perform certain in-office procedures which enable that diagnosis to be made.

If performed, these diagnostic procedures are billed as an additional item charge on your office visit statement. Although these procedures are purely diagnostic in nature, your particular insurance carrier may classify them as “surgical procedures.” If so, the charge may be subject to the surgical deductible of your particular insurance plan. As per the rules of your insurance carrier, you will be responsible for covering any deductible payment.

The providers of Buckhead ENT follow strict federal and state billing and coding guidelines. The most common diagnostic procedures which may fall under your surgical deductible are Nasal Debridment, Nasal Endoscopy, Nasopharyngoscopy, Flexible Laryngoscopy, and Videostroboscopy.
